Access Paediatric Clinic— Privacy Collection Notice

Access Paediatric Clinic (ACN 643 096 753) (we, our, us, ACCESS) is a family-oriented child and adolescent health care service established in 2022. We aim to provide care with honesty and empathy. We aim to reduce the stress on parents and children of presenting to emergency departments, and waiting for very long periods of time for issues which are acute – yet minor enough, which can be managed in a community setting with support from a children’s specialist.

We collect personal and sensitive health information when you (including your child) attend our clinic, make an appointment, or communicate with us through our website, telephone or by email. The personal and sensitive health information we collect may include:

Sensitive Information

  • Medical history, including information about any pre-existing conditions or allergies;
  • Information about family medical history;
  • Information about any current or past treatments or medications;
  • Information about any mental or behavioural health conditions or treatments;
  • Information about any genetic or inherited conditions;
  • Information about any sexual or reproductive health concerns;
  • Information about any chronic illnesses or disabilities;

  • Gender;
  • Insurance information;
  • Medicare information;
  • Emergency contact information;
  • Information about next of kin;
  • Information about medical power of attorney or guardianship;
  • Information about employment and occupation;
  • Information about education and training;
  • Information about income and financial status;
  • Information about race, ethnicity and culture;
  • Information about religious or spiritual beliefs;
  • Information about sexual orientation and gender identity;
  • Information about language spoken at home;
  • Information about immigration status;
  • Information about marital status and family structure;

  • Information about any criminal history; and
  • Information about any other relevant personal information.

We use your personal and sensitive health information to provide you with our services and to communicate with you. We may also use your personal and sensitive health information for research, educational and training purposes, reflective discussions with peers and statistical analysis, and to improve our services. We may also be required to collect, maintain and in some instances disclose personal information and sensitive information to comply with legal or regulatory requirements for collection under and including but not limited to:

  • The Health Records and Information Privacy Act 2002 (SA) (HRIPA)
  • The National Privacy Principles (NPPs)
  • The Australian Privacy Principles (APPs)
  • The Health Insurance Act 1973 (Cth)
  • The Health Practitioner Regulation National Law (South Australia) Act 2010
  • The Children’s Protection Act 1993 (SA)
  • The Disability (Access to Premises – Buildings) Standards 2010; and
  • The Mental Health Act 2009 (SA).

We use your personal and sensitive health information to provide you with our services and to communicate with you. We will use that information for that purpose, and we may also use it to comply with our regulatory and legal obligations, contact you (including by way of email) in response to your enquiries about our services.

We may disclose that information to any third parties whom ACCESS may disclose the information to including other health care providers, doctors, locums, registrars and other authorised healthcare service providers. If the personal information is not collected, we may not be able to provide healthcare services to you.
